SUBMISSION OF NOMINATION FORMS

1. 1. Separate and specific Nomination Forms are available for each Category. Instructions and clarifications are provided, as necessary, in each form.
2. 2. All entries may be submitted in hard copy format and/or online.
3. 3. To allow reported performance by the Nominee to be substantiated and validated, a minimum creative file of one (1) upload or link, for all categories, will be required.
4. 4. Supporting materials may be uploaded in a variety of electronic formats. Please review file format and size requirements to avoid entry problems.
5. 5. Deadline for submission of nomination forms, and payment of entry fees, will be on 29 June 2018. There will be no extensions. An acknowledgment message will be sent for successful submissions.

2. 2. Fill up the Nomination Form online.
3. 3. Read all instructions carefully before filling up the form. All required fields must be filled up.
4. 4. Note the limitations on characters, spaces included, in your answers.
5. 5. It may be advisable to pre-compose your answers to best respond to the information required, yet within the character-input limits specified.
6. 6. Save your data regularly.
7. 7. Prepare photo / media files for uploading to the system.
8. 8. Review the accuracy of data inputted before submitting. Once you have submitted your form, changes to the inputted data will no longer be allowed.
9. 9. Please read the Declaration carefully before signing. Any willful misrepresentation will be grounds for disqualification.
10. 10. Submit online form, with proof of payment, on or before 11:59 p.m. of 29 JUNE 2018 (Philippine Time). Hard copy submissions should be submitted before close of office hours on the same day.
11. 11. Receipt notification will be sent to the e-mail address of both the Nominator as well as the Nominee.
